Family HealthCare Network's PACE Program Reaches Milestone

Family HealthCare Network announced that its Program of All-Inclusive Care for the Elderly (PACE), located at 500 E. School Ave., Visalia, has completed its first year in operation, welcoming more than 100 participants.

PACE is designed to assist participants and their families by providing transportation, physical therapy services, care coordination, social services, meals, an inviting environment and opportunities for participants to connect with their peers.

In its first year, FHCN's PACE program has enrolled more than 100 participants from Tulare and Kings counties. This achievement highlights the critical role of community support, and the positive impact PACE has on families.

Pedro Bautista, the 100th participant, approached PACE with an open mind. As a resident of Farmersville, he was introduced to the PACE program by his daughters, Adriana and Leticia, who discovered it through FHCN.

With transportation provided to and from PACE, it has become much easier for Pedro and his family to manage his care. Now, settling into a new routine, Pedro is thriving at PACE. He shares that the staff are truly kind and treat every participant with compassion and respect. The program has become a meaningful part of his life, offering both structure and community.

The milestone demonstrates how collaborative efforts can lead to significant positive outcomes for families. The program not only provides vital resources and services to its participants but also nurtures new and existing connections that enhance the well-being of the community.
